I hereby certify that all information provided in this application form is true, complete, and accurate. I understand that any false or misleading information may result in the cancellation of my admission without refund. I acknowledge that all documents and credentials submitted shall become the property of the Organiser.

2. Compliance with Terms and Conditions

I agree to comply with all stated and customary rules, terms, and conditions related to the course. I understand that the Organiser reserves the right to terminate my participation without prior notice or refund if I am found to have breached any of these terms or engaged in misconduct.

3. Acknowledgement of Risk

I acknowledge that participation in this programme involves inherent risks, including the risk of injury, disability, or death. Should I observe any unusual or significant hazard during my participation, I will immediately withdraw from the activity and notify the nearest official or course representative.

4. Release of Liability

I hereby release and hold harmless the Organiser, its officers, representatives, agents, and employees from any and all claims, liabilities, damages, or losses, including those arising from negligence, resulting in injury, disability, death, or damage to my person or property, to the fullest extent permitted by law.

I further acknowledge that Hola Wellness Centre acts solely as a co-organiser and collection agent for registration and enrolment fees for this course only, on behalf of the Organiser. Hola Wellness Centre’s role is limited to collecting enrolment details and payments from participants, and it is not involved in the course’s content, instruction, certification process, or operational management.

Accordingly, I agree that Hola Wellness Centre shall not be held liable for any injury, loss, damage, claim, or dispute arising from participation in this course or any related activities.

5. Refund and Exchange Policy

I understand and agree that all payments made for this course are non-refundable once the purchase procedure is completed. Under no circumstances will refunds be provided. However, purchases may be exchanged or replaced in accordance with the policies of the Organiser, subject to applicable terms and conditions.
